How may I view a family member's health record in MyChart?

MyChart - Proxy Access allows a parent or guardian to log into their personal MyChart account and then connect to information regarding their family member.

What is MyChart - Proxy Access?

MyChart - Proxy Access gives parents or legal guardians of minor children access to portions of their child's medical records.
MyChart - Proxy Access can also be used to access portions of your spouse's or dependent adults medical record.
Complete the MyChart Child Proxy Form or MyChart Adult Proxy Form and return it to your provider's office to request access to this convenient service. Before Adult Proxy can be given the MyChart Adult Authorization Form will need to be fill out by the person you are requesting to be proxy for.

Can I ask questions regarding a family member from my MyChart account?

MyChart offers direct access to your personal health record and communicating about another individual's information would be placed in your health record. This information would not appear in the correct health record and could potentially jeopardize medical care.

Can my spouse and I share one MyChart account?

No, due to the sensitive nature of medical information, each adult must establish their own MyChart account.
